INSUFFICIENT EVIDENCE
Used when retrieval quality is too weak/off-topic to answer safely.
SCOPED NAMES
Used only when multi-evidence support passes thresholds; not treated as a complete manifest.
Verification Cadence
Deterministic QA pack runs at fixed UTC times (00:10, 06:10, 12:10, 18:10) and publishes public artifacts at /api/qa-latest.json and /api/qa-history.json.
